Carrier Identifier: The unique identifier for the carrier.

Receipt Date: The day the equipment was physically received.

Report Date: The date the receipt information was entered into the system.  The date may be different (greater than) the date the equipment was actually received.  The system shall automatically populate this field when a new record receipt is recorded.  For revisions the date remains the same as the original.
